If you have heart disease, diabetes, high blood pressure or lung disease like COPD, you may need to make lifestyle changes to stay healthy. In addition to following your physician's instructions about diet, physical activity, and medication, it's also very helpful for your healthcare team to track your overall health status by monitoring objective vital signs and reviewing subjective responses to surveys on a daily basis. With Healthy At Home, our clinical care team can do just that,while you stay comfortably at home.

  • Disease specific patient education (DSMT)
  • Medication Therapy Management and Reconciliation
  • Primary care provider follow-up appointment within 1st week of hospital discharge

Personalized Care
When your physician orders for Healthy At Home, you will be asked to do one or more of the following each day:

  • Check your:
    • weight
    • blood pressure/pulse rate
    • oxygen level
    • blood glucose level
  • Answer questions about how you feel each day and review diagnosis specific education via the telestation

Linkview Health check pictureYour vital sign measurements and answers about how you are feeling are automatically sent to your Healthy At Home nurse daily for review. This way, your nurse will be following your overall health, so whenever there are changes in your condition, she will call you, your physician and other home health providers ensuring effective collaboration and coordination of care.

With this level of monitoring, you can take comfort in knowing that your Healthy At Home nurse will be checking on you daily -- not just during scheduled in-home nurse or physician's office visits. The system alerts your Healthy at Home nurse before a major problem occurs -- potentially helping you stay out of the hospital, prevent a visit to the ER and unplanned doctor visits. 

Your Healthy At Home clinician will deliver everything you need to your home and teach you how to use it. The Healthy at Home monitoring system is very easy to use, so you can take measurements by yourself without a nurse. Results are sent automatically to your Healthy At Home nurse who checks them daily and triages as needed including calling you with additional education.

You take your vital signs in your home, without having to spend the time traveling to your physician's office. The routine takes only a few minutes each day -- at your leisure in the comfort of your own home. In addition, the devices are wireless so they can be set up in your home wherever it’s most convenient for you.

Value Add Program
The Healthy At Home program is available to you at no additional cost. Healthy At Home is provided in conjunction with your core therapies and services such as Home Ventilation and/or Home Infusion for the high risk patients.
